We request the following:

1. All seniors are notified of the availability, criteria and intentions of this scholarship.

2. All applicants must be willing to participate in the annual Chris Maki Violence Awareness walk which will take place on Sunday, April 18, 2004 at 1pm from St. Boniface Church in Lunenburg. (see attached information)

3. Each candidate who may self nominate, or be nominated by teachers or peers submit the following by April:

a) A brief narrative describing themselves and their future plans.
b) Their response to the following question:

In your own opinion why is violence so prevalent today and what action(s) would you suggest to help make a difference.

4. A committee comprised of the Maki Family or their designee; one or two teachers, and a representative of the school administration, shall review the nominations and the narrative and determine the recipient by May.
